Form W-3PR, also known as Transmittal of Wage and Tax Statements, is designed by the US Internal Revenue Service for taxpayers earning income in Puerto Rico. They use it to report financial activity to the Social Security Administration.

What is Form W-3 PR for?

This form is to be submitted strictly when a paper copy A of Form W-2 PR form is being filed. Do not send this form to the SSA or any checks, money orders or cash if you have filed electronically. Also, you shouldn’t file this form alone (without Form W-2).

Is Form W-3 PR Accompanied by Other Forms?

You only need to file it with Copy A of Form W-2 PR.

When is Form W-3 PR Due?

The due date of this form is February 28th, 2017. The taxpayer is required keep a copy of this form for four years.

How do I Fill out Form W-3 PR?

There is a small table to fill out. Check in the box to determine the kind of payer and kind of employer. Provide the correct employer identification number, the employer’s name and address, territorial ID number, contact person and fax number. Then provide the amount of wages, tips and other compensation, income tax withheld, social security wages, social security tax withheld, Medicare wages and tax withheld and social security tips. There are also sections to input non-qualified plans, deferred compensation and income tax withheld by payers of third party sick pay.
